As a Plastic Surgeon, I was trained to be a surgeon….and not an injector. And so when I began practicing medicine several years ago, I focused on surgery and used very little to no facial injectables such as BOTOX Cosmetic. About a year into practice, I decided to give it a shot (no pun intended) and I tried out BOTOX on a select few of my patients. I had very low expectations and thought that my surgical procedures would prove to be far more rewarding and I didn’t see myself using these products very often. What did I find? WOW! I was completely wrong. This stuff really worked.
Fast forward to today and I now have a very busy injectables practice at our Denver plastic surgery office, speak and train on a national basis on the topic of facial injectables, and couldn’t imagine not using them in my practice. What happened? I realized the incredible potential that products such as BOTOX have for not only reversing lines and wrinkles but also for shaping specific areas of the face.
But I still encounter that frequent patient who tells me that BOTOX just doesn’t work for them. So, why is that? To answer this question, let’s look at the 3 reasons why BOTOX will not work for you:
- You are not getting enough units
- You are not being injected correctly
- You have developed antibodies
First, as more and more med-spas pop up seemingly on every corner and as every person with two arms (or maybe less) now considers themself an expert injector, there is bound to be problems. The biggest problem is that many of these spas and salons compete with experienced injectors like myself by lowering their price. And they do so in several ways. Some will simply charge a ridiculous low dollar amount for each unit. Be wary of bargain Botox because you often get exactly what you pay for. Next, some will charge you by the area. This is not a bad practice but the problem is that you don’t really know how many units you’re actually getting. Good injectors will treat you with an appropriate amount of units in each area; less experienced injectors may not. And when they don’t, they can charge you much less per area because you’re simply getting much less per area. How well BOTOX works and how long it lasts has been proven to be directly dependent upon the number of units. Too few units and you simply won’t get maximal results or the results will not last. One injector in my area goes so far as to actually charge by the syringe which is even more ridiculous. With this type of sales approach, while you know the volume you are being injected with, you have no idea how many units this volume actually contains. This is probably the most ridiculous pricing method I have seen and borders not only on deceptive advertising but could potentially be considered an unethical practice, in general.
Next, your injector may not be injecting you correctly. While there are various patterns that people use, some injectors are trained only in patterns and not trained in the underlying anatomy. And when this happens, they inject everyone exactly the same. The problem is that most of us are a little different and probably require a slightly different injection pattern. If the BOTOX doesn’t go where the BOTOX should go, it’s not going to work.
Finally, the last reason that BOTOX may not work for you is that you have developed antibodies. Think about the last time you had a flu shot. You were injected with the viral flu protein, your body recognized it and formed an antibody, and the next time that your body was introduced to the flu virus it already had antibodies and so your immune system was able to fight it off. The same scenario can potentially occur with BOTOX. Over time, your body can develop antibodies and render it ineffective. But the good news is that this is extremely uncommon and most high volume injectors (such as myself) rarely see this happen.
So how do you make sure that the BOTOX you are paying good money for actually gives you the results that you want? The simple answer is that you rely on an experienced injector to give you a reasonable number of units in the proper injection points. While there is never any guarantee, your chance of a good outcome is always better.
